Helios Care in Oneonta receives a grant from the Mother Cabrini Health Foundation to fund a palliative care program for patients who have a chronic disease or face a serious illness and are frequent visitors to the Emergency Department or admitted to the hospital. The grant will fund the Helios Care CHOICES program, which provides person-centered, in-home care and care coordination. CHOICES focuses on managing symptoms, defining personal preferences, and developing plans to best address all of a patient’s care needs.
